Family Works, Inc. is a mid-sized, private, women/minority owned, community based agency, founded in 1990 to provide services to children and families who are involved with the Department of Child Services and Juvenile Probation in Marion, Hamilton, Hancock, Hendricks and Tipton Counties

Home-Based Case Manager Job Description

Salary Range: To be discussed in interview

Start Dates: The hiring date is determined is based on the new hire’s availability.

Licensing: Able to start with a pending temporary license application

Key Responsibilities

Case Managers will provide case management services to the families as approved by DCS/Probation through a family centered and strength based perspective:

  • Case Management Services (budgeting, community referral, parenting education, child development education, stress management, conflict resolution and problem solving, etc.)
  • Supervised visitation
  • Emergency/crisis services
  • Advocate for clients
  • Provide updates to DCS/PO/Court

Essential Functional Requirements

  • Must be able to cope and work within stressful environments
  • Crisis Intervention
  • Maintain productivity standards
  • Conduct initial assessments to determine strengths, protective factors and needs – goal treatment planning based on initial assessments
  • Prepare monthly reports detailing progress towards goals and provide recommendations for  case continuation or closure
  • Adhere to Family Works, Inc. policies, procedures, DCS/PO Service Standards, and operate within the ethical standards of the profession
  • Work Hours - Flexible

Therapists will provide any combination of the following brief therapeutic services to the families as approved by DCS/Probation through a family centered and strength based perspective:

  • Trauma Focused Cognitive Behavioral Therapy (TF-CBT)
  • Alternatives for Families Cognitive Behavioral Therapy (AF-CBT)
  • Cognitive Behavioral Therapy (CBT)
  • Acceptance and Commitment Therapy (ACT)
  • EMDR
  • Providing therapy for Sexually maladaptive or sexually reactive youths – CSAYC
  • Services for victims of sexual abuse
  • Providing sexual abuse assessments and Parenting assessments
  • Addressing family of origin/intergenerational issues, family organization (internal boundaries, relationships, roles, external boundaries, sociocultural history)
  • Self-esteem and Communication skills
  • Grief and Loss Counseling
  • Interpersonal relationships
  • Therapeutic visitation for caregivers and children
  • Adoption issues
