Oaklawn Psychiatric Center is the only faith-based provider for comprehensive mental health and addictions services for children, adolescents, adults and seniors in St. Joseph and Elkhart Counties. We are hiring BSWs and MSWs for case management and clinical positions at our four locations (Goshen, Elkhart, South Bend, and Mishawaka).

Care Facilitator

Care facilitators help children and families in our community live healthier and more fulfilling lives through identifying and supporting their needs and goals. Care facilitators specialize in:

  1. managing the care provided to youth and families, and
  2. helping youth and families gain access to the services they need to meet their goals

Primary duties include:

  • Evaluating the family’s need for services using the Child and Adolescent Needs and Strengths Assessment (CANS) or Adult Needs and Strengths Assessment (ANSA)
  • Developing a plan of care that supports the client’s or family’s goals
  • Helping the client obtain services needed to support its goals, such as food or housing assistance, medical care, Social Security benefits, transportation, skills training, etc.
  • Other duties as assigned

Minimal Requirements:

  • BA/BS required (BA/BS in social service or a human services-related field preferred)
  • Must have a valid driver’s license and proof of insurance
    • Travel within community will occur. Mileage driven will be reimbursed.​​

Skills Training Specialist

Skills trainers help children and families or adults in our community live healthier and more fulfilling lives through teaching skills they need to succeed. These skills focus primarily on activities of daily living and social interaction.

Primary duties include:

  • Identifying needed skills, based on the Child and Adolescent Needs and Strengths Assessment (CANS) or the Adult Needs and Strengths Assessment (ANSA)
  • Working with children or families one-on-one to develop skills such as:
    • Identifying and expressing feelings in a positive way
    • Cooperating with peers and adults
    • Communicating needs in a healthy manner
  • Working with adults one-on-one to develop skills such as:
    • communication
    • organization
    • budgeting
  • help clients put their newly-learned skills into practice through assigned work and encouragement
  • Documenting all client interaction electronically
  • Other duties as assigned

  • BA/BS required (BA/BS in social service or a human services-related field preferred)
  • Must have a valid driver’s license and proof of insurance

Social Worker

Social workers and therapists help children, families and adults in our community move toward health and wholeness through providing individual, family or group therapy.

Primary duties include:

  • Assessing and diagnosing clients
  • Creating treatment plans specific to the client’s goals, strengths and weaknesses
  • Providing individual, family or group therapy interventions that take into account the client’s strengths, weaknesses and preferences
  • Providing crisis intervention and assessing risk factors of harm to self or others
  • Providing timely electronic clinical documentation
  • Consulting and supervision with other clinicians
  • Working collaboratively within a multi-disciplinary team
  • Other duties as assigned​

Minimal Requirements:

  • MSW/LSW required. LCSW preferred. LMFT/LMHC considered.
  • Social Workers would be able to start with a pending temporary license, but we typically ask that they get their license application within 6 months of employment.
