Case Manager - FIS

Salary based on experience.

The Case Manager - FIS is a position that provides services to enable individuals and families to obtain access to needed medical, social, educational, and other support services as well as providing counseling and therapeutic services as specified. The FIS function can be generally described as assessing, advocating, linking and coordinating needed community-based services for the client(s) and families served. The case manager will be heavily involved with and supportive of a client’s educational, legal, vocational, extra-curricular and health related goals, with regular promotion of the accomplishment of these goals. --The FIS generally meets with the client(s) and the client’s family (foster, adoptive or biological) in the client’s home or residence, on a regularly scheduled basis. In addition, the FIS may meet with the members of the client’s community at locations such as school, daycare, job site, etc., as specified to meet the client’s service needs. The Case Manager -FIS’s hours of work will vary depending on the individual needs of his/her caseload but are generally able to be scheduled in a manner that does not exceed 40 hours in a given week.

Essential Duties and Responsibilities Duties:

  • Effectively participate as a member of the team and promote teamwork.
  • Encourage coworkers and work towards building mutual trust, respect, and cooperation among team members
  • Demonstrate good use of time, resources and maintains good organizational skills.  Consistently interact with clients and staff in a professional manner that reflects favorably on the organization and ensures quality care of clients. • Identify and refer clients to resources specific to the client’s needs and issues.
  • Provide case management services for the purpose of assisting clients in behavior intervention.
  • Be prepared to handle crisis situations in the least restrictive way possible and with the greatest amount of consumer participation.
  • Provide ongoing education to client and family members in the form of verbal and/or written communication.
  • Demonstrate accurate use of grammar/spelling and write in clear, concise language.
  • Assist client in scheduling transportation for medical/mental health appointments.
  • Assess client’s living, work, emotional, mental health and social environments, and recommend classes and services to lower stressors in those environments.
  • Identify the developmental needs of clients by helping them to improve their knowledge or skills.

Skills and Knowledge Needed:

  • Ability to evaluate, monitor and support clients’ health, well-being and stability in various life domains.
  • Ability to act effectively in stressful situations.
  • Ability to gather information to make informed recommendations and referrals to appropriate resources.
  • Ability to help clients identify their strengths and needs.
  • Ability to assist with the development, implementation and monitoring of individualized treatment goals.

Qualifications:

  • BSW
  • At least one year of experience working with emotionally disabled persons, education, counseling, etc.
  • Valid driver’s license, proof of automobile insurance.

Clinical Therapist

Salary based on experience

The Clinical Therapist provides individual, group and family therapy, completes psychosocial assessments, safety plans, and treatment plans and participates in interdisciplinary treatment planning.

Essential Duties and Responsibilities:

  • Interviews clients to obtain pertinent intake information and uses clinical assessment measures and techniques to gain understanding of treatment needs and clinical risks
  • Counsels individuals or groups regarding psychological or emotional problems, such as stress, trauma, substance abuse, or family situations, and develops and implements therapeutic treatment plans for the residential setting
  • As applicable to state program requirements, Clinical Therapists may be responsible to screen client referrals and to assist in the placement of clients in foster homes.
  • Plans and administers therapeutic treatment, such as trauma-informed therapy, to assist clients in processing their difficult histories, building healthy relationships with family members, and in gaining better emotion management, social skills, and cognitive processing
  • Responsible for preventing client abuse by adhering to all policies and procedures, attending periodic training, reporting suspicious and inappropriate behavior, and following mandatory abuse reporting requirements Attends court, child-family team meetings, and other important system meetings with client/resident
  • May include providing direct supervision and care for children and youth as necessary
  • May include providing direct clinical supervision to programmatic case management staff • Other duties as assigned

Qualifications:

  • Must be appropriately licensed or certified in mental health and/or have a master’s degree, Ed.D, Ed.S., Psy.D or Ph.D. in the behavioral sciences.
  • An individual with a master’s degree who is on a licensure track and under the supervision of a licensed practitioner is acceptable.
  • Five years of pertinent experience is desired working with children, adolescents and families; any equivalent education and experience.
