Job Description:  The Emergency Shelter Care (ESC) Case Manager oversees the daily operations of the 10 bed ESC unit from intake to discharge. Job responsibilities include intake assessment, coordination of care with the referral source, parents/guardians and the interdisciplinary team, attending court hearings and addressing the needs of all ESC residents through to discharge.  Youth may be admitted from any county in Indiana.

The successful candidate must possess a basic knowledge of principles and practices related to youth development and educational issues. Must have strong and polished interpersonal, written and oral communication skills.  Creative, strategic and analytical thinker with the ability to manage multiple projects. Must be highly organized and able to work well with others.  Able to work with a variety of populations, including adolescents, age 10-18.  Flexibility is required as is the ability to wear different “hats” on short notice.

Travel expectations with job:  You must be willing and able to travel anywhere in the State for court hearings and other necessary appointments.

Work Hours: 8:00am – 5:00pm In addition, you will have on-call and family visit rotation.

Job Description: Arrange for medical and dental appointments and other appointments as indicated. Transport residents to appointments. Obtain provider notes and follow up on doctor’s orders. Review medication administration records, medical concerns and pain assessments daily. Manage medication supply for all residents and administer medications when necessary. Uphold state regulations, residential policies and procedures, and ensure the residents’ rights to safe, prompt and confidential treatment

Job Description:  The Residential Therapist provides assessment and quality therapeutic mental health therapy services to individuals, families and groups within the residential program and the emergency shelter care program as directed. Develops clinical treatment plans and reviews according to standard practices. Completes all clinical documentation at the end of each business day. Engages in weekly clinical supervision with Clinical Director. Works with case managers assigned to youth to ensure co-ordination of services. Consults with referral sources, clinical supervisor and other program staff at least weekly to discuss youth progress. Attends staff meetings to ensure communication, cooperation, and coordination of assigned cases. Maintains customer service standards at all times including while functioning under pressure. Maintains mental health records on a current basis and in accordance with agency standards and state, federal and contract requirements. Maintains resident and staff confidentiality at all times; demonstrates the ability to follow HIPAA policies and procedures. Possesses advanced writing and computer skills to perform the functions of this job description. Provides immediate crisis intervention as necessary. Obtains and maintains appropriate training and continuing education as mandated by licensure body and contract requirements. Conducts self in a professional manner at all times; including refraining from conflict of interest activities and maintaining nondiscrimination policies in all area of practice, regardless of race, age, religion, sex, national origin, disability or veteran status.
