Job Description:

  • Provide clinical services to assigned residents.
  • Conduct all required physical health screenings for residents during the required timeframe. 
  • Make referrals as necessary.
  • Serve as professional nursing link between patient and physicians, pharmacist, case managers, therapists, youth care workers and other care providers.
  • Provide education to residents and staff regarding physical needs, medications, wellness and safety.
  • Dispense and administer medication when needed.  Assess patient compliance and response to medication. Maintain secure storage and appropriate documentation of medication.
  • Provide and assist in ongoing trainings as needed
  • Monitor medication errors.
  • Complete initial health assessments for all Hillcrest residents within the timeframe required.
  • Provide needed therapeutic emergency interventions and protection for individuals who are in crisis.
  • Maintain TB administration certification.
  • Administer TB tests to staff annually as well as residents.
  • Provide nutritional training to all staff and residents as needed.
  • Schedule and attend necessary medical appointments for residents.
  • Participate in treatment team meetings.
  • Participate in Health & Safety meetings.
  • Responsible for supervision of CMA/CNA.

Travel expectations:  Only within Vanderburgh County – no use of personal vehicle required

Work Hours: Monday – Friday 8:00 a.m. – 5:00 p.m. primarily.

Candidate Qualifications:  Associate degree required.  Current, valid State of Indiana nursing license required.  Complete and maintain certification as a TB test administrator. Must be at least 21 years of age. Must have knowledge of relevant clinical issues and demonstrable clinical expertise for resident population served. Obtain and maintain CPR certification and Cornell T.C.I. training. Must meet requirements as identified by State Licensing regulations. Knowledge of medical terminology; excellent grammar, spelling and punctuation. Requires exceptional customer service and phone etiquette; excellent communication skills; attention to detail; ability to maintain effective and organized systems to ensure timely patient flow; skill to act independently and quickly when necessary. Must have valid driver's license, auto insurance that meets the Indiana State minimum coverage requirement, and qualify for Company's vehicle liability insurance. Candidates must pass required background checks including county/state checks, sex offender registry check, and drug screen.

Independent Living/Activities Specialist
(Vanderburgh County)

Salary range: Starts at $15.00 per hour

Job Description:

  • Work with administrative staff to plan programs for Youth Care Workers, stake holders, residents, and families (i.e.:  holiday parties and back to school celebrations).
  • Provide IL case management services.
  • Provide group or individual skill building sessions to address tangible and intangible skills.
  • Assess youth skill level through the online Ansell Casey Life Skills Assessment. Assist in program development while promoting community understanding and awareness of the IL skills program.
  • Meet with each resident within three business days of admission to develop an individualized treatment plan for activities and for IL if age appropriate. 
  • Monitor and complete IL hours for each appropriate resident in a manner to show and ensure compliance with Indiana Code. Complete an accurate monthly summary of all services completed and submit to Director timely. 
  • Assist in TASC prep and attainment.

Travel expectations:  Only within Vanderburgh County – no use of personal vehicle required

Work Hours: Monday-Friday, 9:30 am - 6:30 pm OR 10:00 am - 7:00 pm daily (depending on whether school is in/out of session)

Candidate Qualifications:  High School diploma or GED required.  Bachelor degree in social work or related human service field preferred. Similar work experience may be considered in lieu of education. Must have knowledge of relevant clinical issues and demonstrate clinical expertise for populations served. Excellent communication skills. Must have valid driver's license and qualify for Company vehicle liability insurance. Candidates must pass required background checks including county/state checks, fingerprinting, CPS check, sex offender registry check, and drug screen.
