Family Support Specialist
(Bilingual)

Salary: $15.00

ESSENTIAL DUTIES:
1. Be trained and thoroughly understand the guiding principles of Healthy Families America by attending CORE Training, BASIC Training and all related trainings as requested by Supervisor or Director
2. Provide Counseling to parents during home visits in areas such as: child development, parenting, stress management, crisis management, childcare, effects of child abuse and neglect
3. Responsible to make independent and discretionary decisions for the welfare of the clients
4. Ensure that enrolled clients receive at least 75% of the home visits they are scheduled to receive based upon level of service criteria
5. Provide transportation to families, as needed, for Healthy Families related services
6. Documentation Responsibilities

a) Enter information regarding home visits on a weekly basis into HVTIS and have this information prepared for Supervisor prior to weekly supervision in accordance with supervisory instruction
b) Maintain all contact records and necessary paperwork for client files and enter into HVTIS no later than the 1st of each month for activities that occurred the previous month
c) Complete Family Goal Plan with 90% of families within 90 days of enrollment and subsequent FGP’s in a timely manner as required by program guidelines
d) Complete the DLC with 80% of families within 45 days of enrollment
e) Perform ASQ-3 and ASQ-SE Developmental Screenings on 90% of target children in a timely manner, consistent with Healthy Families guidelines, and repeat the screen in two weeks for failed screenings and give appropriate referrals as required.
f) Refer and link 90% of families to appropriate community resources and document and follow up on these referrals in HVTIS
g) Complete EPDS on 100% of eligible mothers within window prenatally, at 6 weeks and at 6 months. Complete CES-D on MIEC enrolled mothers at 7 months, 12 months, 18 months and every 6 months until 36 months. Complete CES-D on Adult who is not mom and Adult 2 prenatal if enrolled, 6 weeks, 6 months, 12 months and every 6 months until 36 months. Provide appropriate follow up support and referral, based on outcomes of the screening.
h) Complete 90% of the H.O.M.E Inventory within window at 2, 4, 12 and 18 months.  Complete HF Parenting Inventory with 90% of families in a timely manner, consistent with Healthy Families guidelines.
i) Complete IPV Screen on 90% of mothers that enroll and were not screened at Assessment due to lack of privacy. Complete Safety Plans as needed based on outcome of tool.
j) Ensure that 90% of families are linked to a medical provider and record this information into HVTIS
k) Monitor and document in HVTIS that 80% of children are completing basic immunizations and attending well child visits

7. Make 100% of first home visits prenatally or prior to child’s 90-day birthday, unless approved by Supervisory Team.
8. Complete certification for First Aid and CPR
9. Be willing to respond, when off duty, for emergency needs of clients
10. Attend all required Monthly Healthy Families Team Meetings
11. Carry a caseload that meets the standards for Healthy Families Indiana: Full-time Equivalent of 30 points.
12. Schedule and attend weekly supervision: A minimum of 1 hour for staff working less than 35 hours per week and 2 hours for staff working 35 hours or more per week.
13. Attend 4 day HF Core FSS training within 6 months of hire. Complete TLC modules within 12 months of hire, as outlined by HF requirements. Attend all additional trainings as directed by supervisor.

Healthy Families Supervisor

Salary: 31-34k per year

ESSENTIAL DUTIES:
1. Be trained and thoroughly understand the guiding principles of Healthy Families America by attending CORE Training, BASIC Training and all related trainings as requested by Manager or Director
2. Provide community referral sources with information and training on Healthy Families
3. Ensure that all staff complete Healthy Families Indiana required trainings as need
4. Ensure that all staff trainings are documented in the Training Tracking System Database
5. Complete monthly file audits and a quarterly shadow on all staff
6. Responsible for a minimum of 2 hours of individual supervision for each full time staff member and 1 hour of supervision for each part time staff.
7. Documentation Responsibilities:

a) Responsible for all staff supervision logs with weekly supervision documents to include documentation regarding programs, assessment tools, and home visits on each family served.
b) Review quarterly report information in HVTIS
c) Assign cases in HVTIS
d) Review caseload size for all staff, in HVTIS and document this in staff supervision logs
e) Ensure that Quarterly Data Clean-up is completed by SCAN established deadlines.
